Lighthouse Health Group has over 15 years experience in providing comprehensive case management services to people who have sustained a catastrophic injury or have suffered a debilitating illness.

All our Case Managers are Registered Nurses who have extensive experience in working with people within the hospital system and community setting. They are supported in this role by other experienced allied health staff to provide an ‘holistic’ approach to care and assist with the development of a repatriation/rehabilitation plan. Their role encompasses regular liaison with treating doctors, allied health professionals, insurance companies and Area Health Service staff.

The key advantage of our Case Managers having a nursing background is that it assists them to seek information regarding complex medical conditions. Early identification of appropriate services is essential for the provision of cost-effective care and a timely repatriation.

Our services are delivered in a flexible manner and are tailored to meet each individual client’s circumstance. Our Case Managers use a collaborative approach to planning that ensures all key stakeholders are consulted.

In addition to our day to day services, we recognise that clients may require support and advice outside normal working hours.

Our case managers are available 24 hours-a-day

Lighthouse Health Group services clients in both the Sydney Metropolitan Area as well as rural New South Wales. Our Case Managers are also available to offer local service providers with support in order for them to establish and continue to provide medical and rehabilitation services in the client’s own local community.
