Lighthouse Health Group has over 15 years experience in case managing people with complex medical conditions and injuries. Traditionally case management services have been made available to people who have sustained a catastrophic injury or have a disability.

What is Case Management?

In very simple terms case management services assist to identify and coordinate appropriate services and care that will enhance quality of life and independence taking into account individual lifestyle choices, budgets and medical needs.

We recently introduced aged care services into our case management portfolio to offer assistance to families who may have recently acquired the responsibility for coordinating care and/or identifying alternative accommodation or transitional placement facilities for an elderly relative.

We recognise that in today’s world many families have demanding work commitments as well as the responsibility of raising and supporting a young family of their own. Many families are living interstate or overseas which makes it difficult to co-ordinate care or deal with an emergency in an effective way. Time is precious and navigating our complex and ever changing health care system can be stressful and at times locating the right services can be extremely frustrating.

A case manager can assist by providing short or long term assistance to identify and co-ordinate:

• Appropriate support services for your relative in their local area;
  
  
• Alternative accommodation or transitional care facilities when a relatives’ living circumstances change;
  
  
• Transitional care following an acute illness or fall in collaboration with the local General Practitioner.
