Case Management

Our case managers have over 15 years experience in providing comprehensive case management services to children and adults who have sustained a catastrophic injury.

We provide professional services based on a philosophy of ‘family-centred’ care which encourages client/family involvement in the discharge planning process and promotes client empowerment.

Our case managers are supported in their role by other experienced allied health staff to provide a ‘holistic’ approach to the rehabilitation process.

This role encompasses regular liaison with treating doctors, allied health professionals, insurance companies and Area Health Service staff.

The key advantage of our case managers having a nursing background is that this assists them to be able to effectively manage complex medical issues and identify and coordinate appropriate services and care.

Our services are delivered in a flexible manner and are tailored to meet each individual’s circumstance and our collaborative approach to rehabilitation planning that ensures that all the key players in the rehabilitation process are consulted on a regular and on-going basis.

In addition to our day to day services, we recognise that clients and their carers may require support and advice outside normal working hours, particularly when a client first returns to their home and local community.

Our case managers are available
24 hours-a-day for emergency situations at no extra charge!

This extra service provides peace of mind and provides invaluable support until the client is sufficiently confident in their own self-management and care.

Lighthouse Health Group services clients in both the Sydney Metropolitan Area as well as rural New South Wales. Our case managers also offer local service providers with support and training in order for them to be able to establish and continue to provide rehabilitation services in the client’s own local community.
